This clinic is being held indoors at the THURSTON COUNTY PUBLIC HEALTH AND SOCIAL SERVICES BUILDING, 412 Lilly Road N.E., Olympia, WA 98506 10am until 4pm. All minors must receive parent or guardian consent to receive a COVID-19 vaccine. This must be done in PrepMod when registering for an appointment. Minors must have a form of ID confirming their name and birthday in order to receive a vaccination - ID card, passport, birth certificate, MyChart profile, Skyward profile, or insurance information are all acceptable forms of ID. Wear a short-sleeved shirt, but dress with warm layers. Waiting at least 15 minutes after vaccination is recommended.
