This year you MUST receive a booster from a provider that is in-network with your insurance. We are only able to vaccinate you at this time if you have MEDICARE or MEDICAID (except CHPW) or the following commercial plans: Aetna, Asuris, First Choice Health Plan, Humana, Kaiser PPO, United Health Care, HMA, Tricare, UMR. Monkeypox vaccination also offered at this clinic. This vaccine is still provided at no cost by the government, though we are required to collect insurance information as available.

Are a gay, bisexual or other person with a penis or transgender person who has sex with person with a penis AND to whom at least 1 of the following applies: • Had multiple sex partners in the past 3 months. • Had gonorrhea or syphilis in the past year. • Used methamphetamine in the past month. • Attended a bathhouse or public sex venue in the past 3 months. • Homeless or in unstable housing AND staying in a congregate setting. • Exchanged sex for money, drugs or other purposes in the past 3 months. • Incarcerated in the past 3 months. • Black, Hispanic, Latinx, Native Hawaiian, Pacific Islander, Asian, Indigenous, American Indian or Alaska Native.
