Información Adicional: This is a Pfizer vaccination event for anyone age 12 and older. This is a walk in clinic being held at Capital Mall, 625 Black Lake Blvd SW, Olympia WA, 98502 daily from 12pm until mall closing. We will be located inside the mall, closest to the Sephora end of the mall and next to the Buckle store. • All minors 12 to 17 years old must receive parent or guardian consent to receive a COVID-19 vaccine. This must be done in PrepMod when registering for an appointment. Parents or guardians do not have to be present at the time of vaccination if they signed the consent form online during registration. • Please bring a form of identification with you to your appointment - ID card, passport, or other document with your name and birthday on it. • Wear a short-sleeved shirt, but dress with layers if necessary. • Waiting at least 15 minutes after vaccination is required.
